FIRST consent form requirements for both FTC and FRC:

Students/parents, mentors/coaches and volunteers complete the FIRST consent form in their FTC and/or FRC program’s dashboard.

Only the students who filled out the electronic consent form will be on the coach's electronic roster. 

 

FIRST Chesapeake consent form requirements for both FTC and FRC:

 

*PROCEDURE FOR 2021-22 SEASON*

  • All students, coaches, mentors and volunteers are required to fill out the FIRST Chesapeake Consent and Release form before attending any FIRST Chesapeake event. (We no longer collect or accept paper documents.) The online form needs to be completed prior to check-in at the event. The form can be completed here – https://events.firstchesapeake.org/consent-release/ . It is easy to complete via a computer or cell phone. 
  • At check-in the participant will be fitted with a wrist band signifying they have completed and passed the check-in process.

 

 All mentors, coaches, volunteers and students must complete the online form prior to check-in at all events they attend. All team members, including adults must check in at the same time. The status will be checked prior to being given a wristband.
